Uniting in Morwell is seeking an Assessment and Facilitation Officer for a permanent part time role (9 days per fortnight). You will complete holistic assessments of need to support goal directed plans in partnership with service providers to meet individual client needs.

Work is home-based under HACC-PYP guidelines to support younger people with disabilities and chronic conditions, in line with DFFH requirements. You will contribute to care planning and community inclusion initiatives.
