About the Role

As a Senior Case Manager Advocate, you will support older people,



carers and families to navigate the aged care system, access essential services, and remain connected to their community.

You will provide person-centred short-term case management, advocacy and care coordination, empowering clients to build capacity and overcome barriers to achieve their goals.

Working closely with community partners, you will help improve outcomes for older people and promote equitable access to support services.

What You’ll deliver

- Provide tailored case management and advocacy support to older people, carers and families experiencing barriers to accessing aged care and community services.
- Develop person-centred care plans and goals that empower clients to build independence, confidence and community connections.
- Coordinate referrals, outreach support and care coordination meetings to ensure clients receive appropriate services and achieve positive outcomes.
- Assist people with complex needs through challenging times to improve their health and wellbeing
- Build strong and effective relationships with community organisations and stakeholders to improve referral pathways, reduce barriers and enhance outcomes for older people.

What You’ll Bring

- Experience in case management, community services, social work, aged care, mental health or a related field.
- Extensive knowledge of the aged care and community services sector,



with the ability to navigate complex support systems.
- Exceptional communication and relationship-building skills, with a client-focused and compassionate approach, considering cultural preferences and diverse needs.
- Proven ability to advocate for vulnerable community members and address barriers relating to equity and social disadvantage.
- Demonstrated success in working collaboratively with stakeholders to achieve positive outcomes for clients and the community.
- Willingness to undergo pre-employment background checks including National Criminal History Check, Working with Children Check, and Eligibility to Work check.
