Home Caring is seeking an experienced Case Manager to support clients across aged care and disability sectors in Melbourne. The role blends community travel with hybrid in-office coordination, with two monthly trips to Footscray and ongoing engagement in the local community.

You will assess needs, develop person-centred care plans, monitor outcomes, and ensure NDIA/NDIS alignment while delivering compassionate, culturally aware support.
