About Optiform
Optiform is an established multidisciplinary Allied Health practice based in Ballarat, providing evidence-based assessment, support and intervention to individuals and families across Ballarat and wider Victoria. Our experienced clinical team includes Speech Pathologists, Occupational Therapists, Behaviour Support Practitioners and Allied Health Practitioners who work collaboratively to support people across the lifespan.
Our clinicians deliver a broad range of services including early childhood intervention, aged care support, disability support, behavioural support and participation in everyday environments such as home, school, community and workplace settings.

About the Occupational Therapy Role
The Occupational Therapist works alongside the multidisciplinary team to provide clinical occupational therapy services to clients using evidence based clinical practice skills.
The role contributes to Optiform's reputation for clinical excellence, client-centred care and collaborative practice. This role supports high standards of clinical governance, mentoring of developing clinicians and continuous improvement across service delivery, including engaging in a chosen Partfolio. The Occupational Therapist works closely with Directors, Clinical Leads and the Practice Manager to support quality outcomes for clients and the sustainability of the practice.
This position may be tailored to suit early career, experienced or senior clinicians, and operates within Optiform's clinical governance and supervision framework. The Occupational Therapist will ensure that their practice is in accordance with Occupational Therapy Australia Code of Ethics and Optiform Policies and Procedures.
Key Requirements :
- Commitment to ethical, client-centred and respectful practice
- Clinical experience working in a paediatric setting either in early childhood intervention service or the disability sector
- Familiarity and experience administering relevant assessment tools
- Experience providing assessment and intervention for a variety of communication delays/disorders including children with developmental delay, developmental coordination disorder, sensory disorders, Autism Spectrum Disorder,
- Strong interpersonal skills with ability to work within a team, communicate collaboratively with clients, families, and medical professionals
- Demonstrated experience working effectively with children and young people of different ages and abilities as well as diverse cultural and socioeconomic backgrounds
- Eligibility for NDIS Worker Screening; police check; and Working With Children Check
Job Responsibility :
Clinical Practice & Service Delivery
- Deliver services in accordance with Occupational Therapy Australia Code of Ethics and Optiform Policies and Procedures.
- Complete comprehensive assessments and provide direct occupational therapy services to clients.
- Provide a comprehensive treatment intervention and appropriate progression treatment to match functional goals
- Be responsible for completing appropriate administrative duties arising from each session instructed. This requires liaison with the office administration team, reporting of statistical data and completion of clinical documentation.
- Participate in multidisciplinary activities e.g. clinical team meetings, triage meetings, or case conferences, as required.
Client Experience & Communication
- Establish and maintain strong therapeutic relationships with clients and families
- Communicate clearly, respectfully and collaboratively with clients, carers, educators and other professionals
- Provide timely updates, reports and recommendations
- Deliver services using client-centred, trauma-informed and neuroaffirming practice principles
- Respond to client enquiries and concerns within agreed timeframes
Professional Practice & Development
- Maintain current registration, professional memberships and insurance requirements
- Participate actively in supervision, peer consultation and reflective practice
- Engage in internal and external professional development activities
- Incorporate feedback from supervision and review processes into clinical practice
- Adhere to Optiform’s clinical policies, procedures and professional standards
Teamwork & Contribution to Culture

Administration & Operational Responsibilities
- Maintain accurate client records and documentation in accordance with organisational and funding
- requirements
- Complete clinical notes within required timeframes
- Complete assessment reports within agreed deadlines
- Use practice management and communication systems accurately (e.g. Splose, Teams, Trello)
- Manage appointments, cancellations and scheduling responsibilities effectively
Contribution to the Continuation of the Business
- Meet agreed billable service targets appropriate to experience level
- Participate in group programs, school programs or specialty clinics as appropriate
- Contribute to service development, quality improvement and organisational initiatives
- Provide mentoring, supervision or leadership support as agreed
