Our established client is an Aged Care Provider, seeking an experienced Registered Nurse to join their Home Care team as a Care Partner - Clinical.

About the role
Reporting to the General Manager - Home Care, you will manage the full lifecycle of Home Care Packages (HCP) and Support at Home (SaH) services for a diverse portfolio of participants. You will oversee clinical governance, care planning, assessments, coordination of services, and team leadership, ensuring care is delivered in line with individual needs, legislation, and quality standards.
You will also play a key role in developing the Support at Home program, improving service delivery, and supporting continuous improvement initiatives.
Key responsibilities
- Manage onboarding, assessments, reviews, and evaluations for HCP and SaH participants
- Develop, review, and maintain care plans and participant budgets
- Provide expert clinical advice on complex cases, including dementia, palliative and end‑of‑life care
- Lead and support Registered Nurses, Enrolled Nurses, and Support Workers
- Monitor clinical quality, manage risk, and ensure compliance with the Aged Care Act and Quality Standards
- Liaise with participants, families, representatives, and external service providers
- Manage participant feedback and complaints in line with organisational policies
- Ensure accurate and timely documentation in Alayacare
- Contribute to service development, audits,
and performance against KPIs
About you
You are a confident clinical leader with strong community aged care experience, exceptional assessment skills, and a genuine commitment to supporting older people to live well at home.
You will bring:
- Registered Nurse (Div 1) qualification with current AHPRA registration (no conditions)
- Minimum 2 years' recent experience in community‑based aged care
- Strong working knowledge of HCP / SaH Guidelines, My Aged Care, the Aged Care Act and Quality Standards
- Proven experience in case management, home‑based assessments, and budget management
- Demonstrated leadership experience managing and developing clinical staff
- Sound clinical judgement and advanced assessment and decision‑making skills
- Excellent communication, organisation and time‑management skills
- Proficiency in Microsoft Word and Excel; Alayacare experience highly regarded
- Current Victorian Driver's Licence
- Current Police Check and First Aid certification (HLTAID011)
