VMCH appoints a full time Care Partner based in Pakenham to support clients across the Southern Region and Gippsland. You will deliver person-centred care in homes and communities, perform assessments, develop care plans and coordinate services to empower clients and their carers.
You will work within a multidisciplinary team, ensuring quality standards, accurate records and confidentiality while travel as required. A Diploma or higher and Victorian driver’s licence are essential.
