As a full-time Case Manager based in Baulkham Hills with Lifestyle Solutions, you'll play a vital role in supporting children, young people, families, and carers through strength-based, person-centred case management.
You'll work collaboratively to deliver meaningful outcomes by developing tailored support plans, coordinating services, and advocating for the young people in your care.

What You'll Bring
Demonstrated experience in case planning for children and young people, preferably within Foster Care or Out-of-Home CareKnowledge of the Children and Young Persons (Care and Protection) Act ****Experience supporting children and young people who have experienced trauma, disability, or complex behavioursStrong analytical, assessment, and problem-solving skillsExcellent written communication and record-keeping abilitiesAbility to work flexibly and collaboratively with a range of stakeholders

What You'll Be Doing
Manage a caseload of children and young people, working closely with their families and carers to achieve positive outcomesComplete comprehensive assessments and develop, implement, and review individual case plansCoordinate intervention strategies, including risk assessments, referrals, and program applicationsPrepare reports, legal documentation, and support child protection-related court processes where requiredFacilitate access to services and monitor their effectiveness in meeting individual needsAdvocate for children and young people by ensuring their voices, goals, and wellbeing remain at the centre of care planning
