Way beyond simply providing aged care services, we work alongside older Australians to keep choice and control exactly where it belongs, with them.About the roleWe're looking for an experienced Aged Care Service Coordinator to manage a portfolio of Support at Home customers, delivering high-quality care management and service coordination that keeps people safe, independent, and living life on their terms.You'll work in partnership with customers, families, support workers, allied health professionals, and external providers to ensure services are person-centred, outcome-focused, and compliant with the Aged Care Act ****, the Strengthened Aged Care Quality Standards, and the Support at Home Program Manual.What you'll be doingManaging an allocated portfolio of Support at Home customers, from intake assessments through to ongoing care planning and reviewsDeveloping person-centred care plans that reflect customer goals, preferences, and assessed needsMonitoring customer budgets and funding, identifying risks before they become problemsCoordinating service delivery across support workers, allied health, and external providersCompleting assessments and referrals,



including Home Safety and Falls Risk AssessmentsMaintaining accurate, timely documentation that meets legislative and audit requirementsSupporting continuity of care, including hospital admissions, discharge planning, and respite coordinationContributing to a culture of continuous improvement, safety, and inclusionWhat you'll bringCertificate IV in Ageing Support, Community Services, or equivalentMinimum 2 years' experience in aged care, with demonstrated care management experienceWorking knowledge of the Support at Home Program and the Aged Care Act ****Current Australian Driver Licence, National Police Check, First Aid/CPR, and relevant screening checksStrong assessment, care planning, and budget monitoring skillsExcellent communication and documentation skills, with the ability to manage competing prioritiesA Diploma or Degree in Nursing, Social Work, Allied Health, or Human ServicesExperience using My Aged Care and Dynamics 365 (or similar client management systems)You'll be part of a team that genuinely believes the little things count as much as the big ones.
