Our Mission is to promote the dignity, life and spirituality of older people through connected and inclusive communities.We are seeking a compassionate and organised Consultant to assess, coordinate and review the needs of CHSP (Commonwealth Home Support Program) clients and assist them to meet their individual goals with a wellness and reablement approach.
In this rewarding role, you will work with clients, families, referrers and internal teams to support older people to remain independent and connected in their own homes.This is a great opportunity for a client-focused qualified who enjoys a busy, varied role and wants to make a genuine difference in the lives of older Australians through a wellness and reablement approach.The role is Monday through Friday with work times of 8.30am – 4.30pm or 9.00am – 5.00pm.
Initially you will be required to work full time in the office but after training can move to 2 days working from home.
This is parental leave coverage for 1 year finishing July **** with potential to go permanent.Day to day this role will see you:Assessing,



coordinating and reviewing CHSP clients in line with program guidelines and Catholic Healthcare policies and proceduresDeveloping individualised care plans that reflect wellness and reablement approaches and each client's identified goalsResponding to client care matters and feedback within required timeframesEnsuring client My Aged Care profiles are updated to reflect Catholic Healthcare service deliveryInforming and supporting clients, families and carers to understand relevant Catholic Healthcare products and servicesMaintaining accurate and timely documentation and records in Catholic Healthcare systemsWorking in accordance with Aged Care Quality Standards, program guidelines, policies and proceduresSupporting the timely resolution and escalation of client incidents and complaintsProactively assisting clients with referrals for Home Care Package assessments where their needs changeParticipating in team meetings, training and coaching to support ongoing performance and service qualityTo be successful in this role you'll need:Experience delivering telephone-based customer service or client supportA relevant Certificate IV or equivalent qualification in Health, Community Services or a related field would be well regardedStrong time management skills and experience working to targets, processes and competing prioritiesIntermediate to advanced computer skills, including Microsoft Office, databases and client management systemsKnowledge of the community aged care sector and CHSP, or the ability to learn quicklyUnderstanding of the Aged Care Quality Standards applicable to Home Care Services, or the willingness to learnClear written and verbal communication skills, with a warm and professional phone mannerA collaborative, adaptable approach and the ability to switch tasks according to daily prioritiesWhat we can offer you:Not-for-profit tax benefits so you pay less taxFitness Passport discounted gym benefitsCatholic Healthcare is a values-based organisation that welcomes applicants from all faiths and backgrounds.
