We are looking for empathetic and reliable Community Support Workers to join our two homes in Concord.

You will play a key role in helping residents live independently, enjoy their community, and thrive in their daily lives.What you'll be doing:Personalised Care: Providing support based on individual goals and person-centred care plans.Daily Living Support: Assisting with personal care, meal preparation, and essential household tasks.Community Engagement: Facilitating social participation through recreational and leisure activities.Advocacy: Ensuring clients' rights, safety, and wellbeing are protected at all times.Documentation:



Maintaining accurate records in line with NDIS and organisational policies.Collaboration: Working with team members and external stakeholders to deliver coordinated care.About YouWe are looking for someone who is patient, builds trust over time, and is committed to the values of the NDIS.To be successful in this role, you'll bring with you:Experience: Previous experience in community support or disability services.Qualifications: Certificate III or IV in Individual Support/Disability.Safety & Compliance: Current First Aid certificate and a valid Australian driver's licence.Technical Skills: Confidence with medication administration and supporting restrictive practices.Driving: Confident driver (experience with a 12-seater van is a big plus).
