You'll help shape and maintain effective clinical governance frameworks, contribute to accreditation readiness, and build the clinical capability of the team.You're also a key point of support for clinical escalation, helping to identify clinical deterioration early, assisting with the management of complex clinical care needs, and contributing to stronger, more reliable clinical systems.
This role is centred on governance, guidance and assurance, working in close partnership with Community Leaders (Care Managers) and the General Manager.Key responsibilities include:Serve as the most senior clinical expert on site, providing confident and timely judgement for deteriorating, high-acuity and complex residentsLead multidisciplinary case management, bringing together GPs, allied health and specialist services to deliver coordinated, person-centred carePartner with Community Leaders and Registered Nurses to assess,



plan and review care for residents with complex and changing clinical needsLead AN-ACC assessments across the home, ensuring every classification accurately reflects each resident's true clinical acuity and supports funding integrityProactively monitor AN-ACC classification outcomes and work with the care team to close gaps between assessed needs and funded classificationsDrive clinical governance at the home level through audits, quality reviews and continuous improvement planningLead accreditation readiness and support ACQSC audit and assessment processes with confidence and rigourCoach, mentor and educate Registered Nurses and Community Leaders, building clinical confidence and capability across the workforcePartner with the Head of Clinical Governance to implement quality, safety and compliance initiatives at the home levelSkills and experienceSignificant post-registration clinical experience in residential aged care, with demonstrated expertise in complex clinical case managementProven expertise in AN-ACC assessment, classification, funding integrity and documentation standardsStrong knowledge of the Aged Care Act ****, Aged Care Rules **** and the Strengthened Aged Care Quality StandardsDemonstrated experience in clinical governance, quality improvement and incident management including root cause analysisProven ability to lead and support accreditation readiness and regulatory audit processesExperience coaching and developing clinical teams, with the ability to build capability and influence practice without direct line management authorityExperience engaging with the Aged Care Quality and Safety Commission is highly regardedBachelor of Nursing (or equivalent) and current AHPRA registrationPostgraduate qualifications in aged care, clinical leadership or a related specialty highly regarded (desirable)
