This permanent, full time position offers the opportunity to support young people as they transition into employment, helping them overcome challenges, build resilience and achieve long-term success.Working within our Transition to Work (TtW) program, you'll play a vital role in providing ongoing support to participants after they commence employment, helping them maintain sustainable outcomes and reach key milestones.About the RoleAs Youth Support Mentor – Post Placement, you will:Provide structured post placement support to young people who have commenced employmentDeliver regular mentoring, coaching and workplace support to help participants maintain successful outcomesBuild positive relationships with employers, participants and support servicesIdentify and address barriers that may impact employment retention.Support participants through workplace challenges including communication, attendance,



confidence and workplace expectationsCoordinate referrals to internal and external support services when requiredAssist participants to understand workplace rights and responsibilitiesMaintain accurate records and documentary evidence in line with program requirementsWork collaboratively with colleagues and stakeholders to maximise participant success.We're looking for someone who is:Passionate about supporting young people to achieve their employment goalsA strong relationship builder who can engage effectively with participants, employers and community partnersA positive mentor and coach who can inspire confidence and resilienceOrganised, proactive and able to manage competing prioritiesSkilled at identifying challenges early and implementing practical solutionsAdaptable and comfortable working across a variety of environmentsCommitted to person-centred, strengths-based practice.Essential Qualifications & ExperienceTo be successful, you will bring:Experience supporting people in employment or community services, and providing mentoring, coaching and case management supportUnderstanding of person-centred and strengths-based case management, vocational and non vocational barriers affecting employment retention and accessible community support servicesKnowledge of youth development, employment pathways, workplace expectations and employer engagementAbility to identify risks and implement early intervention strategiesStrong customer service, relationship building, written and verbal communication, organisation and time management skillsStrong Digital Identity (MyGovID)Current Driver Licence.Desirable Qualifications and ExperienceCertificate III or IV in Community Services, Youth Work, Employment Services or a related disciplineDiploma qualification in Community Services, Youth Work,



Mental Health or similarExperience within Transition to Work, supporting vulnerable young people with employment retention and workplace interventionsKnowledge of Workforce Australia Transition to Work, Post Placement support requirements, employment legislation, Fair Work and workplace awardsUnderstanding of trauma-informed practice, young mental health and wellbeingStrong cultural capability, including experience working with Aboriginal and Torres Strait Islander young people.Why Join Mai-Wel?
