This permanent part time role is 16 hours per week and focuses on culturally grounded, respectful care, wound care, chronic disease management and patient education.You will join a multidisciplinary team, collaborate with the Diabetes Educator, Dietitian, Social Worker and allied health staff, and build trust with Aboriginal clients and communities to improve treatment
