You'll build strong relationships, deliver person‐centred care, and see the direct impact of your support within the community.What You'll DoComplete home‐based and community assessmentsSupport older adults to remain living independently through assessment, intervention, and preventative careDeliver Functional Capacity Assessments and capacity‐building therapyPrescribe assistive technology and recommend home modificationsProvide therapy that enhances daily living, mobility, and confidenceCollaborate with families, carers, support coordinators, and local health professionalsMaintain clear, high‐quality documentation and communicationWhy This Role MattersEvery visit, every recommendation, and every intervention helps someone stay connected to their home, their routines, and their community.
