Centacare North Queensland is seeking a Case Manager in Mount Isa to provide outreach case management to individuals and families experiencing homelessness or at risk.
You will perform intake, triage and assessments to determine risk and program eligibility, while engaging with multidisciplinar teams and community partners.You will comply with legislation, manage data securely, and contribute to program development and community education.
A driver's licence and relevant checks are essential.
