Community Development and Transition CoordinatorRedcliffe, Brisbane QLDCommunity Development (Community Services & Development)Full-time Fixed-Term Contract to 30 June ****Help shape the future of mental health support in the Redcliffe community.Communify is seeking an experienced Community Development and Transition Coordinator to play a pivotal role in establishing and transitioning the Redcliffe Medicare Mental Health Centre (MMHC) into a thriving, community-connected service.This is an exciting opportunity for someone who is passionate about community engagement, stakeholder collaboration and improving access to mental health services.
Working alongside the Centre Manager and a multidisciplinary team, you'll help ensure the Centre is designed with and for the community through meaningful consultation, co-design and partnership development.About CommunifyCommunify is one of Queensland's leading not-for-profit community organisations, supporting people across all life stages to live healthy, connected and independent lives.Our services span:Mental health and wellbeingAged careDisability and NDISHousing and homelessnessFamily and individual supportAlcohol and other drug recoveryCommunity development and neighbourhood centresOur people are united by a commitment to inclusion, compassion, innovation and social justice.About the RoleReporting to the Centre Manager, you will coordinate community development and transition activities that support the successful implementation and ongoing development of the Medicare Mental Health Centre.You'll work closely with community members, people with lived experience, health providers, government agencies and partner organisations to ensure the service remains responsive, culturally safe and informed by community needs.Key responsibilities include:Developing and delivering community engagement, consultation and co-design activitiesBuilding strong partnerships across health, government and community sectorsSupporting the transition of the Redcliffe MMHC into a full Medicare Mental Health CentreCoordinating stakeholder forums,
