As a full-time Case Manager based in Baulkham Hills with Lifestyle Solutions, you'll play a vital role in supporting children, young people, families, and carers through strength-based, person-centred case management.
You'll work collaboratively to deliver meaningful outcomes by developing tailored support plans, coordinating services, and advocating for the young people in your care.

What You'll Bring
Demonstrated experience in case planning for children and young people, preferably within Foster Care or Out-of-Home Care
Knowledge of the Children and Young Persons (Care and Protection) Act ****
Experience supporting children and young people who have experienced trauma, disability, or complex behaviours
Robust analytical, assessment, and problem-solving skills
Excellent written communication and record-keeping abilities
Ability to work flexibly and collaboratively with a range of stakeholders

What You'll Be Doing
Manage a caseload of children and young people, working closely with their families and carers to achieve positive outcomes
Complete comprehensive assessments and develop, implement, and review individual case plans
Coordinate intervention strategies, including risk assessments, referrals, and program applications
Prepare reports, legal documentation, and support child protection-related court processes where required
Facilitate access to services and monitor their effectiveness in meeting individual needs
Advocate for children and young people by ensuring their voices, goals, and wellbeing remain at the centre of care planning
