Eleanor Duncan Aboriginal Services – Mardi NSW
As an Aboriginal Health Worker within our Social and Emotional Wellbeing (SEWB) team, you will provide culturally safe, holistic support to Aboriginal individuals, families, and community members experiencing a range of challenges that impact their health and wellbeing.
Working within the yadhaba program, you will support clients to strengthen their social and emotional wellbeing through case management, counselling support, care coordination, advocacy, and community education.
You will work closely with families, community members, internal teams, and external service providers to ensure clients can access the supports they need to achieve positive outcomes.

This varied role includes:
Conducting intake assessments and developing case management plansProviding counselling, support, advocacy, and referrals for clients experiencing mental health and AOD challengesSupporting families affected by mental health and substance use concernsCoordinating care and working collaboratively with multidisciplinary teamsBuilding strong relationships with community members and service providersDelivering community education sessions, yarning circles, and wellbeing workshopsMaintaining accurate client records and contributing to program reportingSupporting a culturally safe environment that promotes healing, resilience, and self-determination
Key Responsibilities
Support clients with mental health, social and emotional wellbeing, alcohol and other drug (AOD), housing, and other psychosocial needs.Provide culturally safe case management, care coordination, advocacy, referrals,
and client support.Conduct client assessments and develop individual case management plans in partnership with clients.Provide nontherapeutic counselling and support to Aboriginal clients and families affected by mental health and AOD concerns.Build strong partnerships and work collaboratively with both internal and external services to improve client outcomes.Liaise with Aboriginal Community Controlled Organisations, government agencies, health services, housing providers, and other community organisations to coordinate support.Facilitate groups, workshops, yarning circles, community education sessions, and health promotion activities for adults.Support clients to access health, housing, counselling, welfare, and other services that enhance their overall wellbeing.Work collaboratively with the SEWB and Primary Health Care teams to provide holistic, client-centred care.Maintain strong relationships within the local Aboriginal community to promote early intervention and referrals.Assist with community engagement activities and contribute to the development and delivery of culturally safe wellbeing programs.Maintain accurate client records, case notes, and reporting in accordance with organisational policies and funding requirements.Contribute to continuous quality improvement activities and uphold EDAS values, policies, and Code of Conduct.

What We're Looking For
Certificate IV in Mental Health, Alcohol & Other Drugs, Aboriginal Primary Health Care, Community Services, or equivalent experience (desirable)Experience working with Aboriginal individuals, families, and communitiesDemonstrated counselling, client assessment, and case management experienceStrong knowledge of mental health and alcohol and other drug issuesExperience building relationships with community members and external agenciesExcellent communication and interpersonal skillsAbility to work independently while contributing positively to a multidisciplinary teamCurrent NSW Driver LicenceCurrent NSW Working With Children CheckSatisfactory National Police CheckStrong computer and record-keeping skillsKnowledge of trauma-informed, culturally protected, and strengths-based practiceExperience facilitating community education programs, workshops, or yarning circlesUnderstanding of Closing the Gap priorities and Aboriginal health frameworksExperience working across health, housing, justice, and social support systemsAdvocacy, negotiation, and care coordination skills
