Community Services – Chevron Island QLDCar + Phone + Salary SacrificeWe are seeking a Care Coordinatorwho ismotivated, reliable, compassionate and organised for our Support at HomeProgram (SAH) based in our Gold Coast office.About the Role:The full time positionwillbe responsible for coordinating support services to eligible clients by using a team of in home care staff, clinical and allied health professionals, as well as associated providers.This position includes $1,****** allowance with a fully covered vehicle a mobile phone, plus superannuation and NFP salary packaging.Overall the Care Coordinators' responsibility is to provide quality care services that are compliant with the agency's policy and procedures, program guidelines and the Age Care Quality Standards.
The successful applicantwill be working alongside a team of other Coordinators and liaising with clients, family members and staff to ensure services are provided in accordance with the client's needs,



goals and preferences.Criteria/Qualifications:Minimum Cert IV in Ageing SupportDemonstrated knowledge and experience in a similar role coordinating care services to clientsHave advanced communications skills and the ability to model a positive culture and strong values through a professional serviceHave an understanding and experience in delivering services according to the Aged Care Quality StandardsDemonstrated understanding of the changing and diverse needs of clientsProven relationship management with internal and external stakeholders, as well as good interpersonal and negotiation skillsAbility to communicate in another language would be an advantageCurrent Driver LicenceCurrent National PoliceCertificateissued by the Australian Federal Police CanberraAbout Us:Co.As.It.
