About the Role
Working across the Fraser Coast region, you'll support NDIS participants to:
Implement and maximise their NDIS plansBuild strong support networksNavigate disability, health, housing, mental health, justice and community systemsStrengthen their independence and decision-makingAccess the right services at the right time
This is a hybrid role with flexibility to manage your own diary, work from home, and conduct participant meetings across the region.
You'll be supported by experienced leaders, regular supervision,



and a cooperative team environment that understands the realities of Support Coordination.
We're Looking For Someone Who:
Has previous experience as an NDIS Support CoordinatorIs positive, adaptable and solutions-focusedRemains calm and resilient when supporting complex situationsEnjoys working independently while still being part of a teamBuilds strong relationships with participants, families and providersUnderstands the importance of participant choice, control and dignityHas strong written and verbal communication skillsLives locally within the Fraser Coast region
Essential Requirements
Previous Support Coordination experienceRelevant qualification in Community Services, Social Work, Psychology, Mental Health, Allied Health or a related field (or substantial industry experience)Current Driver LicenceReliable vehicleNDIS Worker Screening CheckWorking with Children Check (or ability to obtain)Ability to satisfy relevant probity requirements
