We are a fast-growing company with support services offered throughout the Moreton Bay, Sunshine Coast and Greater Brisbane Regions and a capacity-building Centre located in Caboolture.
This role involves providing personalised care and assistance to individuals with disabilities to help them live independently and participate actively in their communities.
We are seeking Support Workers experienced in Complex cares of participants.What you'll be doingAssisting with our Day Programs at our Capacity Building Centre, as well as a variety of shifts including Supported Independent Living (SIL) home support, in-home support, and community-based shifts.Assisting clients with daily living activities such as Personal Care, meals preparation, and household tasks.Facilitating and participating in recreational, social, and educational activities to promote client wellbeing.Advocating for client's rights and ensuring they received the support and services they need.Documenting client progress and maintaining accurate records.Collaborating with family members, healthcare professional, and other support services to deliver coordinated care.Adhering to all relevant policies, procedures,
