Permanent full-time position based on the North CoastMonthly Rostered Day Off (RDO)Flexible work arrangements, including work from home opportunitiesPool vehicles availableKilometre reimbursement for approved work travelSalary packaging to increase your take-home payOngoing learning, development and career progression opportunitiesBe part of one of Australia's largest and most respected social purpose organisationsAbout the RoleAs a Case Manager, you'll work alongside young people aged 15 years and over who are preparing to leave care, as well as young adults who have already exited care.You'll provide outreach-based case management that is practical, team-oriented and tailored to each young person's strengths, aspirations and individual needs.
Your role will focus on helping young people achieve sustainable outcomes across:Housing and accommodationEducation and employmentHealth and wellbeingIndependent living skillsCommunity participationFamily, cultural and social connectionsYou'll coordinate services, advocate for young people, develop goal-focused plans, and work closely with Child Safety,
community organisations and other stakeholders to ensure young people have the support they need to thrive into adulthood.This role requires regular travel throughout the service area and offers the opportunity to work independently while remaining well connected to a supportive and experienced team.What You'll BringWe're looking for someone who is passionate about supporting young people and confident building strong, trusting relationships.You will ideally have:A relevant Degree in Social Work, Psychology, Human Services, Social Science or a related discipline (or equivalent experience)Experience providing case management to young people with complex needsStrong engagement, assessment, planning and advocacy skillsExperience coordinating services and working collaboratively with internal and external stakeholdersExcellent communication and documentation skillsThe ability to work independently, manage competing priorities and maintain accurate case recordsAn understanding of trauma-informed and strengths-based practiceExperience supporting young people with a care experience will be highly regardedA current unrestricted Queensland Driver LicenceSuccessful Candidates Will RequireNational Criminal History CheckWorking with Children Check (Blue Card)Licenced Care Service Check (LCS)Applications will be reviewed as they are received, and the vacancy may close earlier than the advertised closing date.
