Based across Mount Druitt and Blacktown Community Health Centres, you'll deliver person-centred care within community and home-based settings, supporting some of Western Sydney's most diverse communities.This is an exciting opportunity to make a meaningful difference by helping clients remain secure, active and connected in their own homes and communities.Work alongside experienced physiotherapists, occupational therapists, nurses and other allied health professionalsDeliver innovative care through home visits, community-based services and telehealthBe part of a supportive team focused on collaboration, learning and clinical excellenceAccess opportunities for professional development,



clinical supervision and quality improvement initiativesContribute to improving health outcomes for older adults living with chronic and complex conditionsAs a Chronic & Complex Physiotherapist Level 1/2, you will provide evidence-based physiotherapy assessment, treatment and education to older adults with chronic, complex and age-related health conditions.
Working autonomously and as part of a multidisciplinary team, you will support clients to restore function, improve mobility and achieve their individual goals.Deliver comprehensive physiotherapy assessments and interventions in clients' homes and via telehealthDevelop, implement and review goal-focused care plans that promote independence and physical wellbeingAssess falls risk, functional capacity and other factors impacting safe community livingCollaborate with multidisciplinary teams, carers and external service providers to coordinate holistic careParticipate in quality improvement activities, professional development and evidence-based practice initiativesRequirementsQualification: Tertiary qualifications in PhysiotherapyRegistration: Current general Physiotherapy registration under AHPRASelection CriteriaDemonstrated experience and skills in Physiotherapy assessment,



planning and evaluating interventions for clients with chronic and complex health conditions.Demonstrated understanding of the health and associated care needs of persons with complex, chronic and/or aged conditions residing in the community.Demonstrated ability in the use of relevant evidence based, best practice guidelines to guide decision making for best client outcomes.High-level written and oral communication skills, computer competence and ability to use electronic medical records, word processing and databases.Ability to work independently and as part of multidisciplinary team.Current Drivers Licence valid within Australia.People with disabilities who meet the selection criteria are encouraged to apply; and where required, WSLHD will implement reasonable adjustment consistent with industry standard.Aboriginal and/or Torres Strait Islander people are encouraged to apply.
