Frontline Support Services is seeking a dedicated and experienced Case Manager to join our team in Cairns, QLD.
This is a subcontracting role (ABN required), offering an prospect to make a meaningful impact in the lives of children, young people, and families within our community.As a Case Manager, you will play a key role in coordinating and delivering essential support services to vulnerable individuals and families while working collaboratively within a supportive and professional team environment.Key ResponsibilitiesConduct comprehensive assessments to identify client needs and develop tailored support plansCoordinate a range of support services, including referrals, advocacy, practical assistance, and stakeholder engagementAdvocate on behalf of clients to ensure appropriate supports and resources are accessedWork collaboratively with internal teams and external stakeholders to deliver holistic, client-focused careMaintain accurate and detailed case notes and documentation in line with organisational policies and compliance requirementsParticipate in regular supervision, team meetings, and ongoing professional developmentParticipate in an afterhours on-call roster shared within the case management teamAbout YouA relevant tertiary qualification in Social Work, Psychology, Human Services,
