You will travel daily, conducting comprehensive assessments and connecting clients with the right services to maximise independence and quality of life.This agile, community-based role involves meeting clients where they are, delivering support when needed most, and contributing to a multidisciplinary team.
AHPRA registration and a valid driver's licence are essential.
