The role is centred on delivering high-quality, participant-focused support, building strong relationships, and assisting participants to achieve their goals and access the services they need.What You'll Be DoingSupporting participants to understand and implement their NDIS plansCoordinating services and connecting participants with suitable providersBuilding strong relationships with participants, families, and stakeholdersPromoting independence, choice,



and capacity buildingMonitoring budgets and ensuring NDIS compliancePreparing reports and supporting plan reviewsAdvocating for participant goals and positive outcomesWhat You'll BringExperience in NDIS Support Coordination and/or case managementStrong understanding of the NDIS and relevant frameworksExcellent communication and relationship-building skillsStrong organisational and time management abilitiesQualifications in Disability, Social Work, Psychology, Allied Health, or a related field (desirable)Confidence using Microsoft Office and case management systemsNDIS Worker Screening CheckWorking With Children Check (WWCC)National Police CheckCurrent Driver's Licence and reliable vehicleWhy Join Concept Care?
