Our Parkinson's Nurse Specialist's provide up to date, evidenced based information and support services to people living with Parkinson's and other movement disorders, their carers and family within a community setting.About the roleTHRFG Parkinson'sare seeking anexperienced Registered Nurse to join our dedicated team as a Parkinson's Nurse Specialist.
This role is a highly autonomous 0.6 FTE position where you will provide individualised support to people in the community living with Parkinson's Disease.
This position will deliver specialist clinical care including case management, advocacy and provision of clinical education within their scope of practice.Reporting to the Nurse and Healthline Lead, you will:Deliver advanced, evidencebased clinical nursing care and case managementExercise highlevel autonomous clinical judgement and decisionmaking within scope of practiceContribute to clinical standards, protocols and service improvement within a communitybased, nurseled modelCoordinate and prioritise care using advanced clinical reasoning, including timely referrals across multidisciplinary and community servicesDeliver,



monitor and evaluate therapeutic interventions and support programsCollaborate within multidisciplinary primary health care teamsAdvocate for clients and carers to support equitable access to health, Aged Care and NDIS servicesBuild and maintain strong community and professional networksProvide culturally safe, inclusive care that promotes access and equityDeliver education to health professionals, clients, families and stakeholdersMaintain advanced clinical expertise through ongoing career development and a contemporary practice portfolioContribute to research, education resources, data collection and quality improvement activitiesAbout youAHPRA Registered Nurse with a minimum 5 years' post graduate experienceExperience and or interest in chronic disease management and movement disordersAn understanding of primary and tertiary health careAbility to work effectively both independently and as a member of a multi-disciplinary team with a flexible positive attitude.Why us?
