ABOUT THE ROLEIf you're looking for a meaningful role where you can see the impact you make every day join our skilled, supportive, values driven multidisciplinary team based in Warabrook.
As a Clinical Care Coordinator, you'll build lasting relationships with people living with complex mental health and psychosocial challenges and support them to navigate the health system, build on their strengths, and work towards their recovery goals.
Enjoy flexible part-time, school-friendly hours, a mix of office based and community outreach work and opportunities for professional growth within our evolving mental health services.DUTIESUndertake comprehensive and ongoing biopsychosocial assessments to identify, monitor and meet diverse individual needsDevelop collaborative care plans in partnership with clients, families/carers, formal and informal support networksProvide evidence-based clinical care coordination, treatment and psychosocial supportProvide person-centred, trauma-informed and culturally safe careCoordinate care with GPs, psychiatrists,



allied health and community mental health servicesProvide assertive outreach to support transitions and reduce avoidable hospital presentationsSKILLS & EXPERIENCEDegree qualifications in a mental health profession (nursing, social work, or occupational therapy) with current AHPRA registration and/or relevant professional body accreditationMinimum 3 years' clinical experienceExperience in community-based mental health service delivery, including care coordination and/or crisis interventionsAbility to work autonomously and as part of a team within a strong clinical governance framework, and build effective stakeholder relationshipsHighly developed communication and interpersonal skillsCurrent NSW Working with Children Check clearanceRecent or new (employer facilitated) Criminal History Check.
