RSL LifeCare Newcastle is seeking a Care Coordinator to support our Home Care team as we grow, delivering services to veterans and seniors in NSW.
You will manage a portfolio of clients, develop care plans and coordinate services to meet individual needs.The role requires experience in home care packaging (HCP), CHSP, DVA, budgeting and solid liaison with external providers, with CPR and a valid driver's licence preferred.
Police/NDIS checks are required.
