As we continue to strengthen our Aged Care at Home services across Queensland, we are seeking an experienced and values-driven Clinical & Care Partnering Manager to join our team in Far North Queensland (based in Cairns).
This is a key leadership role within our new Service Region model, focused on delivering high-quality, person-centred care and strong clinical governance across the region.Your RoleAs the Clinical & Care Partnering Manager, you will lead and oversee the delivery of clinical care and care management services in the community, ensuring safe, consistent and high-quality outcomes for clients.Working closely with the Service Region Manager and multidisciplinary teams, you will provide clinical leadership, strengthen service integration, and support continuous improvement across the region.Key ResponsibilitiesClinical Leadership & Service DeliveryProvide operational leadership for the delivery of evidence-based clinical services and care managementOversee practice in clinical assessment, care planning and ongoing reviews aligned to client goals and preferencesSupport best practice clinical interventions including chronic disease management, wound care and palliative carePromote a person-centred, coordinated approach to care across multidisciplinary teamsEnsure services meet Aged Care Quality Standards, legislative requirements and organisational policiesLead clinical risk management, including incident oversight, investigation and continuous improvement actionsMonitor clinical governance frameworks and implement strategies to enhance safety and quality of careConduct audits,



reviews and reporting to support compliance and accreditationProvide professional guidance, mentoring and support to clinical, allied health and care management teamsFoster a culture of collaboration, accountability and continuous learningBuild strong relationships with internal stakeholders and external partners to enhance service coordinationOperational OversightEnsure accurate and compliant clinical documentation and reportingSupport service planning, performance monitoring and continuous improvement initiativesContribute to regional service strategies in partnership with the Service Region ManagerAbout YouYou are a confident clinical leader with a strong commitment to person-centred care and quality outcomes.
