About the Opportunity
TheCare Services Coordinator (Clinical Operations)is a SCHADS Level 4 leadership position designed for an experienced Enrolled Nurse who wants to make a broader impact within disability services.
Working under the clinical governance of ourCare Service Manager (Registered Nurse), you will provide a critical link between:
Participant outcomes.
Workforce capability.
Quality and compliance.
This role provides an opportunity to transition your nursing skills into a community-based leadership environment while remaining connected to frontline practice.
You will support participants, coach disability support teams and contribute to improving service quality across the organisation.

Key Responsibilities
Clinical Coordination & Enrolled Nurse Practice
Apply Enrolled Nurse knowledge and clinical judgement within your scope of practice.
Support implementation of participant health plans, clinical care plans and allied health recommendations.
Monitor participant wellbeing and identify changes requiring escalation.
Support high-intensity daily personal activities where trained and competent.
Assist with clinical coaching, competency development and workforce education.
Promote safe, evidence‐based and person‐centred practices.
Operational Leadership & Workforce Support
Coordinate safe and effective participant service delivery.
Provide day‐to‐day leadership and guidance to Disability Support Workers, Senior Disability Support Workers and Team Leaders.
Coach and mentor employees to strengthen practice capability.
Support participant transitions and changing support requirements.
Identify operational risks and contribute to practical solutions.
Provide operational input into rostering, workforce capability and continuity of care.
Quality, Compliance & Continuous Improvement
Support NDIS Practice Standards compliance.
Review documentation, incidents and participant support records.
Assist with incident follow‐up and improvement actions.
Promote safeguarding, participant rights and positive outcomes.




Contribute to quality improvement initiatives.
On-Call Operational Support
The role participates in a rotating after‐hours On‐Call roster, providing:
Operational advice and support to employees.
Assistance with participant and service delivery concerns.
Incident escalation and coordination support.
Effective communication and handover of operational matters.
About You
You are an experiencedEnrolled Nursewho combines clinical capability with solid communication, leadership and relationship‐building skills.
You will have:
CurrentEnrolled Nurse (Division 2) registration with AHPRA
Experience supporting people with disability, complex care needs or vulnerable populations
Understanding of person‐centred practice and safeguarding principles
Ability to coach, mentor and support frontline teams
Strong communication and documentation skills
Ability to manage competing priorities and work autonomously within established frameworks
Experience in the following areas will be highly regarded:
Supported Independent Living (SIL).
High‐intensity daily personal activities.
Complex disability support.
Clinical coordination.
Team leadership or workforce mentoring.
NDIS quality and compliance systems.

Current AHPRA registration as an Enrolled Nurse (Division 2).
NDIS Worker Screening Clearance.
National Police Check.
Working with Children Check (where applicable).
Current First Aid Certificate.
Current CPR Certificate.
Current Australian Driver Licence.
Evidence of unrestricted right to work in Australia.
