Disability Support Worker (Experienced/High Support)Ability Choice Care – Blacktown Westpoint NSWAt Ability Choice Care, we are more than a NDIS provider—we are a team passionate about empowering people with disability to live meaningful, independent lives.As our organisation continues to grow, we are looking for adaptable, experienced, compassionate, and resilient Disability Support Workers who are committed to delivering exceptional person-centred support.We have a variety of shifts available across Greater Western Sydney, including:Active overnight shiftsMorning, afternoon, and evening shiftsCommunity accessSupported Independent Living (SIL)About the RoleYou will provide high-quality, person-centred support to participants with a diverse range of disabilities and personal needs, helping them achieve their NDIS goals while promoting independence, dignity, and choice.Your responsibilities may include:Personal care and daily living supportMedication Management- monitoring and reportingCommunity access and social participationMedication assistanceEmotional and mobility support to participants with complex physical and psychosocial needsWorking with participants who display challenging behaviours.Implementing Positive Behaviour Support strategiesImplementing and observing authorised restrictive practices in accordance with legislation and NDIS Practice StandardsCompleting accurate progress notes and incident reportingSupporting participants to build independence and life skillsWorking collaboratively with families, advocates, and allied health professionalsWho We're Looking ForWe are looking for support workers who are confident,
resilient, and enthusiastic about delivering quality care.You will ideally have experience supporting participants who may rely on you for one or more of the following requirements.High-intensity supportMental health supportAssistance to navigate environmental and social challenges.
E.g., Persons on the autism spectrum who encounter a range of situational and learning challenges such as anxiety, communication/expression, oppositional behaviour.Advocacy and connection with services/clinicians e.g., persons with an intellectual disability who need an intermediary.Hoist, transfer and manual handling assistance.Behaviour support and restrictive practicesSupport to manage and recognise symptoms of chronic health conditions: e.g. Epilepsy, diabetes,Experience working within Supported Independent Living (SIL) is highly regarded.An experienced management team will support you.A large portion of our services includes overnight support.Successful applicants must be willing to work:Active night shiftsPublic holidays on a rotating rosterFlexibility is highly valued.To be successful, you will have:Minimum 3-5 years' disability support experience (preferred)Certificate III or IV in Individual Support, Disability,
or equivalentMandatory NDIS Modules x 3Current NDIS Worker Screening CheckWorking with Children CheckCurrent First Aid & CPRCurrent Driver LicenceFully insured and reliable vehicleExcellent written and verbal communication skillsAbility to work independently and within a teamHighly DesirableApplicants with knowledge and experience in any of the following will be highly regarded:High Intensity Support SkillsMedication AdministrationRestrictive PracticesPositive Behaviour SupportMental HealthComplex Behaviour SupportPEG FeedingDiabetes ManagementEpilepsy ManagementTrauma-informed practiceWhy Join Ability Choice Care?
