This full time position is based in Atherton and will require travel to outer regions as part of our SHS team.What you'll be doingProvide early intervention services to individuals and families to maintain tenancies and avoid entering homelessnessConduct assessments and develop individualised support plans with clientsProvide practical support to help clients develop financial and independent living skillsLiaise with relevant government agencies, community organisations and stakeholders to coordinate support and referral servicesParticipate in team meetings and contribute to the continuous improvement of our SHS servicesWhat we're looking forExperience in the community services,
