About the Role
As aChild and Family Practitioner, you'll play a key role in supporting children and young people in family-based care arrangements to achieve safe, stable and positive outcomes.
Working collaboratively with foster carers, children, families, Child Safety and other key stakeholders, you'll coordinate and deliver high-quality case management while providing ongoing support, supervision and guidance to carers.

This position is based in ourSpringwoodoffice and is offered on a6-month max-term contract.
Key Responsibilities
Deliver high-quality, child-focused case management, planning and assessments.
Partner with Child Safety, carers, families and other stakeholders to develop and review individual care plans.
Provide ongoing support, coaching and supervision to Foster Carers through home visits, phone contact and regular reviews.
Support carers to implement case plans and achieve positive outcomes for children and young people.
Provide supervision and support to Youth Workers where required.
Develop and monitor risk assessments, cultural support plans, safety plans and behaviour support plans.
Maintain accurate and timely case notes, foster care agreements, renewals and records within organisational systems.
About You
You'll be passionate about improving outcomes for children and young people and enjoy building strong relationships with carers, families and stakeholders.
You'll also have:
A recognised degree qualification inSocial Work, Psychology or a related discipline
Demonstrated experience within the child protection and/or out-of-home care sector
Experience working from a strengths-based, trauma-informed and culturally responsive framework
The ability to assess, support and develop foster carers while promoting permanency and positive outcomes for children
Excellent communication, organisational and relationship-building skills
A current Australian Driver Licence
Successful candidates will be required to complete probity checks including a National Criminal History Check and Working with Children Check (where applicable).
