Gladstone Community Linking Agency (GCLA) is seeking an experiencedAged Care Care Partner (CHSP)to join our growing Aged Care team.

About the Role
As anAged Care Care Partner (CHSP), you'll work closely with older Australians, their families and carers to coordinate services that promote independence, wellbeing and quality of life.
You'll manage a caseload of participants, develop and review individualised care plans, coordinate services, monitor participant wellbeing and ensure services are delivered in line with funding requirements and the Aged Care Quality Standards.
This is a rewarding role for someone who enjoys building relationships, solving problems and helping people achieve the outcomes that matter most to them.
Key Responsibilities
Complete participant assessments, care planning and regular reviews.
Coordinate services that support participants to remain living safely at home.
Work closely with participants, families, carers and service providers.




Monitor participant wellbeing and respond to changing support needs.
Maintain participant budgets and ensure services align with available funding.
Maintain accurate documentation and participant records.
Promote participant choice, independence and dignity.
Identify and elevate safeguarding, compliance and service delivery concerns.
Contribute to continuous improvement and quality outcomes.
About You
We're looking for someone who is compassionate, organised and committed to providing exceptional person-centred care.
Certificate IV in Ageing Support, Community Services or a related qualification (or equivalent experience).
Experience in aged care, care coordination or case management.
Excellent communication and relationship-building skills.
Robust organisational and time management skills.
Ability to work independently and collaboratively.
Current Australian Driver Licence.
Current First Aid and CPR Certificate.
Working with Children (Blue Card)
NDIS Worker Screening Check (Yellow Card)
Experience working within the Commonwealth Home Support Program (CHSP), Home Care Packages or Support at Home.
Knowledge of the Aged Care Quality Standards and My Aged Care.
Experience using client management systems.
