Your part in our UnitingCare team You'llsupportpeoplelivingwithdisability to live meaningful lives through person-centred planning, community connection and coordinated administrative support.
The role contributes to the delivery of high-quality, compliant services byfacilitatingindividual plans, supporting daily living outcomes, and undertaking administrative and record-keeping functions that enable effective service delivery.
You will: Provide support to people in their everyday living situations, including personal care asidentified, with a strong emphasis on choice, enhancing quality of life,healthand wellbeing, whilst ensuing duty of care is always upheld
In conjunction with program leadership - develop, implement, and review individual plans of the person being supported.
Assistpeople to fulfil valued community roles through engagement with community groups, potential employers and suitable programs or activities.
Advocate for andfacilitatewarm referrals tosuiableservices to address client needs outside the scope of the program.
Undertake a range of administrative functions that directly support service delivery and individual outcomes,including:basic financial and transactional activities,data capture,



documentation and reporting related to case management,monitoringand supporting compliance with organisational,fundingand legislative requirements.
Undertake other administrative duties such as reception back up whenrequired Contribute to an setting of excellence and drive for positive outcomes

Relevant certification anddemonstratedexperience in theDisabilitysector Awareness of legislation underpinning the provision of services within theDisabilitysector and the key issues affecting quality of life for people with a Disability A current NDIS Worker screening check
Current CPR and First Aid Certificate (requiredbefore you start)
