As an Aboriginal Health Practitioner/Worker within the Aboriginal Chronic Care Unit, you'll join a small, dedicated team passionate about improving the health and wellbeing of Aboriginal people living with chronic conditions.
Working collaboratively with healthcare providers across ISLHD, you'll help Aboriginal people navigate the health system, access coordinated care, and connect with the services and supports they need throughout their healthcare journey.
You'll build trusted relationships with individuals, families and communities, providing culturally safe, person-centred care that empowers people to better understand and manage their health closer to home.
Through advocacy, care coordination and strong partnerships, you'll play a key role in improving access to healthcare, enhancing patient experiences, and contributing to better long-term health outcomes for Aboriginal communities.

Working With Children Check (WWCC)
Holds a Certificate IV in Aboriginal Primary Health Care (Practice) and a current authority to practice as an Aboriginal Health Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA), or holds a minimum Certificate III qualification in Aboriginal Primary Health Care or equivalent
Valid NSW Drivers Licence
