NSW Health's Sydney Local Health District is seeking an Aboriginal Health Worker to support the YUDI 48 Hour Follow Up Care Coordination Service at Redfern Community Health Centre.
This role focuses on culturally secure post-discharge follow-up for Aboriginal and Torres Strait Islander people with chronic conditions to improve continuity of care.
The ideal candidate will hold or be working toward a Certificate III in Aboriginal Primary Health Care, have lived experience in Aboriginal communities,
