About the Role
We are seeking a mature, professional, and compassionate Disability Support Worker to provide person-centred support to a NDIS participant who lives independently in her own home.
The Participant has excellent communication skills, enjoys maintaining family and community connections, and is committed to living as independently as possible. She requires support with managing day-to-day life matters, maintaining her home environment, attending appointments, community participation, and coordinating practical aspects of her life.
This role would suit someone who has a genuine interest in empowering people to maintain their independence, exercise choice and control, and achieve their personal goals.
About the person
The Participant is an independent and engaging individual who communicates well and is actively involved in her supports. She values her autonomy and benefits from practical assistance to help manage appointments, household responsibilities, community access, and other life administration tasks.
An important aspect of her life is maintaining connections with family, including regular visits to her mother in aged care.
Key Responsibilities
Provide person-centred support in line with her NDIS goals.
Assist with managing appointments, scheduling, and important life administration tasks.
Support to manage her home and maintain a secure and organised living environment.
Assist with community access, social participation,



and transport as required.
Support to attend appointments and activities.
Facilitate family and community connections.
Encourage independence, decision-making, and self-advocacy.
Liaise professionally with service providers and other stakeholders as required.
Maintain accurate progress notes and documentation.
Experience in disability support, community services, mental health, aged care, case management support, or related fields.
Current Police Check (not older than 3 months)
Current NDIS Worker Screening Check.
Current First Aid and CPR Certificate.
Valid Australian Driver Licence.
Reliable and insured vehicle.
Excellent communication and organisational skills.
Ability to work independently and exercise sound judgement.
Strong professional boundaries and a person-centred approach.
Highly Desirable
Experience supporting people who are subject to Public Guardian arrangements or working alongside the NSW Public Guardian.
Experience with advocacy, service coordination, and helping participants navigate complex service systems.
Experience supporting individuals to maintain independent living arrangements.
