About Us

The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people's homes, other community venues as well as virtual care (telehealth).
An exciting opportunity is available to join Primary Care and Community Health (PCCH) as a Project Officer within the Community Counselling Service. This part-time position (3 days per week) is funded for three years through the NSW Health Refugee Adaptable Fund and will support the implementation of the First **** Days Pathways for Refugee Families Project.
This project aims to improve the health, developmental and psychosocial outcomes of refugee and asylum seeker families during the critical first **** days of a child's life. You will work in partnership with Primary Care and Community Health (PCCH) services, culturally and linguistically diverse (CALD) families,



and representatives from CALD community organisations to build a contemporary understanding of the health and psychosocial needs of refugee and asylum seeker families within NBMLHD.
The role will focus on building strong stakeholder partnerships, co-designing strategies that improve access to maternal, early child and family health services, and strengthening community-based supports for families. You will contribute to the review and development of models of care and service pathways, identify barriers to engagement, support service improvements, and promote coordinated, trauma-informed and culturally safe care.
You will also work with staff across the District to understand challenges in delivering care to refugee families and develop initiatives that build workforce capability and confidence. In addition, you will contribute to project planning, implementation and evaluation activities, ensuring outcomes align with NSW Health priorities and evidence-based practice.

What you will bring to the role
Degree in Social Work from an accredited course providing eligibility for membership of the Australian Association of Social Workers
Demonstrated ability to initiate, lead, complete and share quality improvement initiatives and service evaluation processes.
Demonstrated clinical experience working effectively with multicultural clients and communities with young children and a comprehensive understanding of the social, cultural, and structural vulnerabilities that influence their health and wellbeing.
Demonstrated excellent negotiation, decision making and advocacy skills including the ability to interact constructively and collaboratively with consumers and a diverse range of internal and external stakeholders, throughout all organisational levels.
Demonstrated effective communication skills and ability to provide high level clinical advice from an education and training perspective and support to others in providing service excellence in a variety of clinical and non-clinical settings
Demonstrated high level organisational, time management, and prioritisation skills, with the ability to manage competing demands and deliver outcomes within required timeframes
Proven capacity to work effectively both independently and as part of a multidisciplinary team in a dynamic, multicultural project environment.
Current NSW State driver's licence (including P1 or above) and willingness to use for work purposes. This may include driving long distances across the LHD
