Hunter Primary Care is seeking a Service Mapping & Intake Officer to join the Spoke service in the Hunter region.
You will triage referrals, match clients to appropriate care, and conduct risk assessments to ensure timely, secure access to services.
Work includes building relationships with GPs and Local Health Districts, maintaining a current directory of regional services, and guiding clients to the right level of care quickly and compassionately.
