About the role
The Community Health Registered Nurse is an integral part of the multidisciplinary healthcare team providing direct nursing care to patients, both in the community setting and Ambulatory Care setting.
This includes clinical interventions, monitoring or complex medical care that does not require hospitalisation.
The role includes leading, directing and supervising other nurses and other carers in the team, such as ENs, students and Aboriginal Health Workers to support quality care for patients.
The role works closely with the Hospital in the Home, Chronic Care, Wound & Stoma Care and community based services to support holistic and patient centred care that assists people to live well at home, avoid preventable hospital admissions, and supports earlier safe discharges.

This role will require a mix of day, afternoon and weekend shifts.
