About the role
Deliver high-quality, evidence-based nursing care to heart and lung transplant, heart failure, pulmonary hypertension, mechanical circulatory support and respiratory patients within a specialist outpatient clinic
Assess, plan, coordinate and evaluate patient care, including triage, outpatient therapies and infusions, vaccinations, patient education,



and the coordination of planned and urgent admissions
Collaborate closely with a multidisciplinary team, including Clinical Nurse Specialists and external healthcare providers, to ensure safe, timely and seamless continuity of care
Provide expert clinical support by monitoring patient conditions, responding to clinical changes and patient enquiries, and coordinating referrals to optimise patient outcomes
Contribute to service excellence through leadership, quality improvement initiatives, professional development, and a commitment to delivering safe, person-centred care in line with best practice standards
About you
Currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency
Demonstrated ability to work within the interdisciplinary team using effective communication and interpersonal skills
Demonstrated effective time management skills and ability to prioritise competing workloads within a complex environment
Demonstrated evidence of integration of theoretical knowledge and clinical skills, critical thinking and analysis in your daily practice
Demonstrated commitment to ongoing professional development
