Key Responsibilities include:
Document patient admissions (inclusive of a best possible medication history), conducting clinical medication reviews, andcollaboratively formulating medication management plans.
Exercise independent judgement and problem-solving skills, in matters beyond routine practice, using valid and reliable evidence, with infrequent clinical practice supervision.
Perform the role with a high degree of independence with infrequent clinical and management supervision.
Act as a key clinical reference for geriatric pharmacotherapy, providing expert advice to medical, nursing and allied health staff on safe and judicious use of medicines in older adults.
Remuneration value up to $******* p.a,
