Communify is seeking a Pathways Facilitator to join the Medicare Mental Health Centre in Redcliffe. You will be one of the first points of contact for participants, families and referrers, coordinating referrals with empathy and trauma-informed care.
You will handle enquiries, build rapport, manage referrals in Redicase/PMHCIS, and maintain accurate records while supporting service targets and cooperative teamwork.
