This is an exciting opportunity to join Ku-Ring-Gai Neighbourhood Centre Ltd (KNC) as a full-time Care Partner for Support at Home. In this key role, you will be responsible for overseeing the delivery of high-quality care services to our clients, ensuring their needs are met in a compassionate and productive manner. The position is based in St Ives, NSW. Full time and part time positions available.
What you'll be doing
Developing and implementing care plans to meet the individual needs of each client
Supervising and supporting a team of care workers to deliver exceptional client care
Liaising with healthcare professionals, families and other stakeholders to coordinate holistic care
Monitoring and evaluating client progress, making adjustments to care plans as needed
Ensuring compliance with all relevant healthcare regulations and standards
Implementing quality improvement initiatives to continuously enhance our clinical services
Contributing to the overall strategy and growth of the organisation
What we're looking for
Bachelor's degree in social or Community or a related field
2+ years' experience in a care management role, preferably in the community healthcare sector
Excellent communication and interpersonal skills, with the ability to collaborate effectively with diverse stakeholders




A background in working with clients who have complex needs
Strong clinical expertise and a deep understanding of best practices in client-centred care
Proven track record of leading and developing high-performing teams
Exceptional problem-solving and decision-making abilities
Commitment to upholding ethical standards and providing compassionate care
