About The Organisation
We are currently seeking experienced Support Workers to join our growing team across the Perth metropolitan region.
We have a range of shifts available, including morning, afternoon, active night and sleepover shifts.
Please indicate your availability and preferred shift type in your application.

Key Responsibilities
Support participants to work towards achieving their individual goals in line with their support plans
Assist with daily living activities, including personal care, cooking, cleaning, shopping and community access
Support participants to build confidence, independence and life skills
Assist with medication administration in line with organisational policies and training
Implement positive behaviour support strategies
Encourage participation in age-appropriate activities, employment and social engagement
Maintain a safe, respectful and supportive environment at all times
Monitor participant wellbeing and report any concerns to the Manager
Accurately complete required documentation, progress notes and communication records
Participate in individual planning processes and monitor outcomes
Liaise with families, carers, case managers, allied health professionals and community agencies
Work in line with NDIS standards, WHS guidelines, privacy and confidentiality requirements
Collaborate with team members to ensure consistent and high-quality service delivery
Skills & Experience
Qualification preferably in community/disability/individual support/aged care/mental health and/or relevant experience




Experience working with clients with challenging behaviours
Well-developed verbal and written communication skills
Current Driver Licence and reliable vehicle
100-point ID check
Valid visa/working rights in Australia

Mon to Sun- 6AM to 2PM & 2PM to 10 PM
