Western NSW LHD is seeking a Clinical Nurse Consultant for the Specialist Dementia Care Program (SDCP) in Western NSW. This Contract/Casual role focuses on expert leadership and consultancy to support older people with complex dementia and mental health needs.
The appointment involves in‐reach care coordination, coordinating secure transitions, and strengthening clinical pathways to deliver person‐centred, evidence‐based care across community, specialist and residential settings.
