Join Our Team as a Care Partner (casual) - Support at Home Program
Are you passionate about empowering older Australians to live safely and independently at home? Adventist Retirement Plus (ARP) home care is seeking a dedicated Care Partner to join our Support at Home Program in Yeppoon. About the Role
As a Care Partner, you will lead the planning, coordination, and monitoring of individualised care management across a range of home care services. You will work closely with participants, their supporters, and interdisciplinary teams to ensure the delivery of tailored, responsive, and inclusive support. This role is key to fostering autonomy, safety, and wellbeing for our clients.
Key Responsibilities
Develop and coordinate individualised care plans, including annual reviews and holistic health planning
Monitor changing needs and update care plans accordingly
Coordinate service access, referrals, scheduling, and provider liaison
Assist clients with budget management and financial statements
Support care transitions, including respite and post-hospital admissions
Build and maintain solid relationships with participants, families, and external providers
Advocate for clients and promote supported decision-making
Maintain accurate documentation and ensure compliance with aged care regulations
Engage in continuous quality improvement and customer feedback initiatives
Qualifications & Experience
Diploma in Community Services, Aged Care, Case Management, Nursing, or equivalent experience (preferred)
Certificate III or IV in Individual Support (Ageing),



Aged Care, Disability, or Community Services
Experience in care coordination, case management, or similar roles in aged care, health, or disability services
Strong knowledge of Support at Home Program and Aged Care Quality Standards
Excellent communication, empathy, and cultural competence
Proficiency with care management or CRM systems and Microsoft Office
Consent to a criminal record check (police check) or NDIS Worker Screening Check
Current Driver's License, First Aid, and CPR certificate
Experience with My Aged Care portal and relevant networks (desirable)
Must have valid working rights in Australia.
Consent to a pre-employment health/functional assessment.
