Veterans' Home Care (VHC) Assessor
.

About the Role
This is an ideal opportunity for someone looking to step away from physically demanding frontline care while continuing to use their experience to support others.
As a
Veterans' Home Care Assessor
, you'll conduct structured telephone assessments with eligible veterans and war widows/widowers, identifying their needs and helping them access practical in-home supports that enable them to remain independent.
You'll spend your day building rapport, listening carefully, asking thoughtful questions and documenting assessments, knowing that every conversation contributes to improving someone's quality of life.
You'll be supported with comprehensive onboarding, ongoing training and a collaborative team environment where questions are encouraged and professional development is genuinely valued.
About You
We're looking for someone who genuinely enjoys helping people and is comfortable balancing meaningful conversations with detailed administration.
You will ideally bring:




Minimum two years experience in aged care or community services
Experience supporting older Australians in community, residential or health settings
Strong communication skills with the ability to build rapport over the phone
Excellent attention to detail and accurate documentation skills
Confident computer skills, including Microsoft Office and working across multiple systems
A calm, empathetic and professional approach
Australian Citizenship (essential)
Current National Police Check (or willingness to obtain)
